The Wagner Transitional Care neighborhood is designed to accommodate individuals who are recovering from illness or surgery and may require rehabilitative services including physical, occupational, speech and aqua therapies as ordered by a physician.

The Extended Care neighborhoods are available for individuals with
on-going health concerns. Various opportunities are offered to promote spiritual, emotional and physical wellness including music, cooking, intergenerational groups, exercise groups and special interest programs.
Special Care...
The Special Care neighborhood offers specialized services for those with Alzheimer's and related conditions who also require Nursing Care. A safe and secure environment promotes individuality and dignity and offers group and individualized activities that maximize independence and provide appropriate stimulation.
Rehabilitative Services
Rehabilitative Services offers physical, occupational and speech therapies to restore and rehabilitate individuals to their highest level of independence.
